Measure the stoma size from the base using the provided measuring guide. Trace the selected stoma outline on the release paper of the skin barrier. Ensure the small starter hole is in the center of the tracing. (Consult with healthcare professional regarding gloving). Cut the center hole to match the stoma size using curved, short-blade scissors. Remove the release paper from the skin barrier. Place the skin barrier around the stoma, holding it for 30 seconds to ensure a snug fit. After placing the center disc, remove one side of the release paper from the tape collar and smooth it onto the skin. Repeat with the other side, ensuring smooth and wrinkle-free application of the tape. For colostomy/ileostomy with a drainable pouch, close the pouch before application using the "tail closure" resembling a large "twist tie." Remove the paper from the adhesive strip on the tail closure. Position the closure 1/4 inch from the open end on the outside of the pouch. Press in place. Fold the pouch tail tightly around the closure four times. Fold the closure ends toward the center on the side of the pouch closest to the skin (flange side), ensuring a smooth and flat tail without crimping. For urostomy pouches, close the drainage tube by bending the outlet spout and pressing the plug fully into the spout. Ensure the inner surfaces of the pouch are separated and there is some air inside. Position the pouch flange over the barrier flange. Starting from the bottom, apply gentle pressure around the flange circumference, listening for audible "clicks" indicating a secure lock. Always double-check the coupling for complete closure. To remove the old pouch and skin barrier, use a wet paper towel or soft washcloth to clean the skin. Hold the pouch edge with one hand and gently push down on the skin with the other hand to release the skin from the pouch and skin barrier. Remove them slowly and gently together.
